Patient Policies.

Are there any additional fees?

There is a $50 administrative fee for lengthy, time-consuming forms (for example, disability forms) for non-membership patients, and a $50 administrative fee for copying or faxing medical records for non-membership patients transferring to another practice. These fees are waived for membership patients.

How often may I see the doctor as a membership patient?

You may see the doctor in the office as many times as medically necessary as a membership patient. In addition, the doctor is available 24/7 via e-mail, text, and personal cell phone.

Do I need to be a membership patient to be seen?

We are a membership based practice and prioritize those patients. Self-pay medical patients may be seen on a case-by-case basis.

Are you accepting new patients?

We are currently accepting new membership patients. Please note, we reserve the right to decide who we see as a patient in our practice.

Do you take walk-ins?

We currently do not accept walk-ins. Appointments are required so that we have ample time to verify your demographics.

What about HMO prior approvals?

If your plan is an HMO that requires authorizations to be done by in-network physicians, we would not be able to do this — you would need to see the primary care doctor to whom you were assigned in order to get an authorization. This typically applies to specialist referrals, imaging studies, physical therapy, home health care, and similar services.

What is your payment policy?

Payment in full is expected for services rendered, and outstanding balances must be paid before requesting additional services such as appointments or medication refills. We are not a lending institution and do not offer payment plans; outstanding accounts are sent to collections. If your membership is terminated for any reason, including non-payment, a $50 administrative fee is charged in addition to any outstanding balance.

We keep a credit card securely on file — similar to hotels and car rental agencies. After your appointment, any remaining balance is charged to the card and a receipt is e-mailed to you.

What is your policy on pain medications?

Due to changes in federal and state legislation regarding narcotic pain medications, we do not routinely prescribe narcotics from this office nor do we manage chronic pain.

What is your policy on medication refills?

Refill requests are processed as time permits during regular business hours — please plan accordingly. Refills for controlled substances require a mandatory office or telemedicine visit as regulated by the Florida Department of Health.

Do you provide women's services?

We do not provide routine women's services such as Pap smears or gynecologic exams and procedures.

Do you see children?

Please note we are not pediatricians and do not see children. We do not provide routine well-child visits, vaccinations, or sick-child visits.

Do you offer vaccines?

We do not administer vaccines at this time. They are available at most major pharmacies through your insurance.
